How to register with Disability Services

  1. Be admitted as a student with UA - Pulaski Technical College and have a current class schedule.
  2. Schedule an appointment with the Office of Disability Services by calling (501) 812-2738 or stop by Campus Center Building, Room 241. At Little Rock-South, call (501) 812-2862 or e-mail [email protected].
  3. Meet with a disability services counselor to discuss how your disability is affecting you in the classroom and discuss appropriate academic accommodations.
  4. Submit documentation of your disability during the appointment. Documentation provided after the appointment may delay the approval of some or all requested accommodations.

Documentation

Do not delay meeting with a disability services counselor because you are concerned that you may not have appropriate documentation. If documentation is needed, we can assist you in obtaining documentation from a qualified diagnostician or from colleges you previously attended.

Learning Disabilities Documentation

An individualized education program with psycho educational evaluation testing scores along with functional limitations, recommendation of accommodations, diagnosis and signature of a diagnosing professional.

ADD/ADHD Documentation

Letter from a primary care physician stating the diagnosis and the functional limitations of the disability, along with recommendation and conclusion ADD/ADHD evaluation completed by an appropriate diagnosing professional.

Psychological Disabilities Documentation

Psychological evaluation with diagnosis and recommendations and conclusion from appropriate professional.

Medical, Physical and Sensory Disabilities Documentation

Primary care physician or referring physician documentation that is appropriate to the functional limitations of the disability.
