Instructions:
1. Ask your instructor(s) if they plan to provide your testing accommodations. Test proctoring through Disability Services is offered as an optional service for instructors and your instructor may choose to administer your final exam with your approved accommodations.
If your final exam is scheduled outside of Disability Services office hours (Monday-Friday, 8 a.m. – 4:30 p.m.) and your instructor plans to have you test in Disability Services, discuss an alternative testing time during Disability Services office hours and submit this form. To schedule appointments for 8 week terms, contact the Disability Services office at (501) 812-2862.
2. Inform your instructor if you have back-to-back exams needing extended time accommodations and discuss an alternative time for taking the exam.
3. Submit this form if you are taking your final exams with Disability Services. Please contact the Disability Services office if you need assistance completing this form.
4. Submit this form by the posted deadline for the semester. Students submitting after the posted deadline will not be guaranteed testing proctors.
