Please complete this form for an individual (non-UA-PTC student) providing services to UA - Pulaski Tech as a volunteer (not including camps). Volunteers may not provide services to the college until a background check has been completed and approved. An e-mail will be sent to the volunteer's personal e-mail address provided on this form from the Office of Human Resources that will provide information needed to complete their background check.
