The intent of this form is to provide an opportunity for staff to communicate suggestions and concerns with Staff Senate.

Please read the following information before completing the form:

  • Any communications sent through the web form will be visible by the Staff Senate Officers in order to best review your questions and comments.
  • You may remain anonymous, but if you wish to have a response, please include your contact information.
  • If you are requesting a response back, please allow us time to investigate your concerns.
  • Please keep in mind another department may need to respond if your question or concern is outside Staff Senate’s purview. We will not contact others on your behalf but instead will provide guidance to you on who best to contact.
  • Issues, ideas and concerns raised anonymously will be brought to Staff Senate during its meetings, and discussion of the comments will be reflected in posted meeting minutes.
